Official Birth Certificate, Passport, or State-Issued ID
Immunization
Physical Exam
Transcript, Report card, Record Summary of Academic History
Current IEP or 504 plan (if applicable)
Verification of guardianship (birth certificate, Court Custody Documentation, Educational Guardianship Forms)
